Secondary Service Connection: The Condition Your Condition Caused

VA disability law recognizes that one condition can cause or aggravate another. A secondary claim asks the VA to rate a condition that was caused or made worse by a condition that's already service-connected.

How the chain works

You have a service-connected knee injury (DC 5257, other impairment of the knee). Over years of favoring that knee, you develop a limp that loads your opposite hip and your lower back. That altered gait can support secondary claims to the hip and the spine. Our Rating Code Navigator shows these chains for curated codes with the current 38 CFR Part 4 citations.

Two rules govern the chain

  1. Secondary claims still need the three elements: a current diagnosis, medical evidence linking it to the primary condition, and a competent medical opinion connecting them (the "nexus" — at least as likely as not).
  2. The anti-pyramiding rule (38 CFR 4.14): you cannot be paid twice for the same symptoms. The knee pain itself is rated once — the genuinely separate, additional disability the knee caused is what a secondary claim covers.

Common chains worth knowing

(Educational examples — verify with your representative.)

  • Knee → altered gait → back/hip (5257 primary; spine under 5242/5243 family for IVDS)
  • PTSD → sleep apnea (9411 → 6847)
  • Service-connected injury → chronic NSAID use → gastrointestinal issues (drug-induced secondary)
  • Tinnitus/hearing loss isn't itself a mental-health rating, but the functional effects are rated separately per the mental-health criteria if medically evidenced.

Look up your codes and their chains in the Rating Code Navigator. Then take what you learned to a free accredited VSO.

Frequently Asked Questions

What is a secondary service-connected condition?

A condition that was caused or made worse by a condition that's already service-connected. A secondary claim asks the VA to rate that second condition.

What is the nexus for a secondary claim?

A competent medical opinion connecting the two conditions — at least as likely as not. Along with a current diagnosis and medical evidence linking it to the primary condition, it's one of the three elements every secondary claim needs.

What is the anti-pyramiding rule?

38 CFR 4.14 — you cannot be paid twice for the same symptoms. The same symptom cannot be rated twice under two diagnoses; a genuinely separate and additional disability can be rated on its own.
